Transcranial Magnetic Stimulation (TMS) for Obsessive Compulsive Disorder (OCD)
Recruiting
Transcranial magnetic stimulation (TMS) is an FDA-approved, non-invasive form of brain stimulation for OCD. TMS has been shown to be an effective form of treatment in individuals with OCD. By using a form of TMS termed theta-burst stimulation (TBS), we hope that this will result in a more effective treatment by producing faster symptom reduction. We are trialing a novel form of accelerated TMS, where we will deliver ten 10-minute sessions per day, for 5 days. This study will compare the efficacy of TMS for OCD at two distinct brain regions. Participants will be randomized into one of two study groups, and receive stimulation at either the DMPFC or R-OFC. Both regions have been linked to OCD by research. Patients that don't respond to their study region will have the option to receive stimulation at the other region.

Mucosal Impedance Sleeve Gastrectomy
Recruiting
Obesity is a growing epidemic and bariatric surgery has been shown to be the most effective and efficient mean of achieving significant and sustainable weight loss in morbidly obese individuals. Studies have demonstrated that after LSG, between 5 and 30% of patients suffered from GERD, with a small subset of those patients (2.9 %) converting to another bariatric procedure (gastric bypass) because of debilitating GERD symptoms.
The investigators propose this current study to examine mucosal impe... Read More

Gracie Diet for Gastroesophageal Reflux Disease
Recruiting
The goal of this clinical trial is to learn if dietary changes can help improve gastroesophageal reflux disease (GERD) symptoms. The main question\[s\] it aims to answer whether the Gracie Diet is an option to treat GERD symptoms in individuals wish to discontinue standard doses of PPI and / or H2 receptor antagonists.
Participants will be taken off PPI and be placed on the Gracie Diet for 8 weeks. Information about the participants reflux symptoms and GERD health related quality of life will b... Read More

After defining the manometric characteristics of UES incompetence associated with documented pharyngeal reflux, we will determine the reproducibility of manometric criteria for UES incompetence in prevention of pharyngeal reflux. We hypothesize that these criteria are comprised of either a single or constellation of manometric abnormalities. After determining the ability of externally applied cricoid cartilage pressure in preventing pharyngeal reflux, we further hypothesize that this approach wi... Read More

Barrett's Esophagus Related Neoplasia (BERN) Project
Recruiting
* The use of high resolution endoscopy (HRE), narrow band imaging (NBI) and chromoendoscopy increases the detection rates of Barrett's esophagus (BE) and early neoplasia.
* Endoscopic mucosal resection (EMR) will improve the accuracy for detection of dysplasia/early neoplasia.
Specific Aim 1 - To create a video-atlas of non-dysplastic and dysplastic/early neoplastic lesions in patients with BE. This will be used for training purposes and to assess learning curve associated with these new techno... Read More
