Skin irritation can also occur, and this is where flushing needs a careful explanation. Flushing is a sudden warmth, redness, and tingling of the skin, most often on the face and upper body. It is mainly associated with nicotinic acid, the niacin form of vitamin B3.6-9 Nicotinic acid triggers flushing through its effect on blood vessels.
This matters because flushing should not be blamed on “NAD+” as a whole. Nicotinamide, another vitamin B3 form, does not cause flushing, because its structure works differently in the body.6-9 NR and NMN are not the classic flushing forms either. NR (nicotinamide riboside)10 has been studied in short-term human trials and is generally reported as well tolerated at the doses tested, with stomach symptoms, headache, and lightheadedness among the reported possible effects.9 Long-term safety is not established.
NMN (nicotinamide mononucleotide)11 has also been examined in short-term studies with tolerability broadly reported as acceptable, though the body of human safety evidence is smaller and shorter than many people assume.12 Again, long-term data are limited.
NADH,13 the reduced form of the coenzyme, has less human safety data than NR or NMN. Because the evidence base is thinner, there is simply less to say with confidence about its side effect profile.
Nicotinamide (niacinamide)14 is a vitamin B3 form that generally causes fewer side effects than nicotinic acid and does not cause flushing.6 However, at very high doses, vitamin B3 forms including nicotinamide can carry serious risks, covered in the next section.6

A concrete example is the NR-SAFE trial. In that study, NR at 3,000 mg per day for four weeks was reported as well tolerated, with no moderate or severe adverse events attributed to the supplement.7 That sounds reassuring, but two cautions matter. First, the study authors themselves stated the data do not guarantee long-term safety.7 Second, this trial had industry ties, so it should not be read as independent proof that high-dose or long-term use is safe for the general public.7 One short, industry-linked study is a data point, not a verdict.

If you have liver disease, the liver-related risks of high-dose vitamin B3 forms are the concern.8,27 A clinician can weigh your liver status against any product you are considering. NAD+ IV Therapy and Injection Safety
IV or injected NAD+ carries route-specific risks and has limited outcomes evidence, and oral supplement data do not prove IV safety or effectiveness. These are two different safety lanes. What is known about swallowing a pill does not transfer to receiving an infusion into a vein.
On regulatory status, IV and injected NAD+ are compounded sterile drug preparations, not FDA-approved products.32 Compounding is governed by the 503A and 503B frameworks under the Drug Quality and Security Act, which set out how compounded preparations may be made.32-34 Sterile-compounding and cGMP standards are meant to reduce contamination and dosing errors. Quality signals worth checking include PCAB/ACHC or NABP accreditation, verifiable state licensure, and a disclosed pharmacy partner.35
On evidence, a 2026 systematic review found no eligible outcomes trials of IV or intramuscular (IM) NAD+ for anti-aging or wellness use.22 In plain terms, the controlled evidence to support these infusions for those purposes is not there.
Clinics that offer NAD+ drips have reported route-specific discomforts during infusion, including sensations like chest tightness, nausea, or cramping, which are often managed by slowing the drip.
