NAD+ for Anti-Aging: What the Research Says
NAD+1 is promoted for anti-aging through supplements, injections, and intravenous infusions. There is real biology behind NAD+, but the science supporting these products is more cautious than the claims made online.
Nicotinamide adenine dinucleotide, or NAD+,1 is a coenzyme2 found in every cell. A coenzyme is a substance that helps enzymes carry out chemical reactions. NAD+ supports energy metabolism, DNA repair, and other core processes, and its levels decline with age.3 Current human research does not prove that NAD+ supplements reverse aging, extend lifespan, or make people biologically younger.4-6
That distinction matters. A product may raise the amount of NAD+ or related compounds measured in blood without improving health, function, or survival.4 Blood measurements are biomarkers, meaning measurable signs of a biological process. They are useful, but they are not the same as a clinical benefit.
This article separates what NAD+ does in the body from what oral supplements, IV therapy, and injections are proven to do. It also explains safety concerns, product quality, and lower-risk ways to support NAD+ metabolism.

Supplement Regulatory Status
Oral NAD+, nicotinamide mononucleotide (NMN), and nicotinamide riboside (NR) products are sold as dietary supplements, not approved drugs. The U.S. Food and Drug Administration does not pre-approve dietary supplements for safety or effectiveness before sale, although federal labeling and current good manufacturing practice, or cGMP, requirements still apply. “Not FDA-approved” does not mean “unregulated.” Independent verification through USP or NSF is a meaningful product-quality signal, but it does not prove that a supplement provides anti-aging benefits.7,8

Why NAD+ Matters in Aging Biology
NAD+ matters because cells need it to turn nutrients into usable energy and to support enzymes involved in DNA repair, inflammation, and mitochondrial function.2,3 NAD+ levels decline with age, which makes restoring them biologically plausible as an aging strategy. Plausibility is not proof that a treatment slows aging in people.

During energy metabolism, cells break down carbohydrates, fat, and protein. NAD+ accepts and transfers electrons during these reactions, helping cells produce adenosine triphosphate, or ATP, the main chemical form of cellular energy.
Much of this work occurs in mitochondria, the structures inside cells that produce most ATP. When NAD+ availability changes, mitochondrial activity and other metabolic pathways may also change.2,3
NAD+ is also used by sirtuins, a family of enzymes involved in gene control, stress responses, metabolism, and inflammation. Sirtuins require NAD+ to function, which is one reason researchers have studied the relationship between NAD+ and aging.9,10
Human tissue studies have found age-related differences in NAD+ and related metabolites, while animal and cell experiments have identified possible causes. These include greater NAD+ use, impaired recycling, mitochondrial stress, and increased activity of CD38, an enzyme that breaks down NAD+.11-14
This creates a plausible model: lower NAD+ may weaken energy metabolism, DNA repair, and stress responses, which could contribute to age-related changes. Animal experiments support parts of this model, but they cannot establish that an NAD+-raising product prevents aging in humans.
The evidence ladder helps explain the uncertainty:
- Cell studies show how NAD+ affects isolated cells and molecular pathways.
- Rodent studies test whether changing NAD+ affects health or lifespan in animals.
- Short-term human studies measure NAD+ biomarkers and basic metabolic changes.
- Randomized human trials test symptoms, function, or disease-related outcomes.
- Long-term outcomes studies determine whether an intervention prevents disease, preserves healthspan, or extends life.
Healthspan is the period of life spent in generally good health. NAD+ aging research has substantial cell and animal evidence, plus short-term human biomarker data. It does not yet have convincing long-term human evidence showing longer life or a younger biological age.
What Human Research Shows About Oral NAD+ Precursors (NR, NMN, NAM)
Oral NAD+ precursors reliably raise NAD-related biomarkers in humans, but evidence for anti-aging outcomes is mixed and inconclusive. The findings support claims that certain products can affect NAD+ metabolism. They do not establish that those products slow or reverse aging.
Most clinical studies use NAD+ precursors, which are compounds the body can convert into NAD+. These include:
- Nicotinamide riboside, or NR
- Nicotinamide mononucleotide, or NMN
- Nicotinamide, or NAM, a form of vitamin B3
Researchers focus on precursors because intact oral NAD+ is broken down during digestion and metabolism, while precursor pathways can supply materials used to rebuild NAD+ in tissues.9,10
Human studies show clear biochemical target engagement. This means the intervention changes the biological target it was designed to affect. NR trials have reported increases in blood NAD+ or related metabolites after supplementation.15,16
NMN has also raised NAD-related metabolites in human trials. In one randomized study of postmenopausal women with prediabetes, NMN increased muscle insulin sensitivity, but several other metabolic measures did not change.17 This was a finding in a specific group, not proof of general anti-aging benefit.
A 2026 systematic review found consistent biochemical target engagement across human studies but heterogeneous outcomes, meaning the studies differed in their participants, products, doses, length, and measured results. It concluded that NAD+ precursors can alter biomarkers, while evidence for meaningful aging or wellness outcomes remains inconclusive.18
A 2024 randomized controlled trial in people with mild cognitive impairment provides a useful example. Mild cognitive impairment means a measurable decline in memory or thinking that does not fully prevent independent daily life. NR increased blood NAD+ and was well tolerated, but it did not improve cognition.19
Other randomized trials have reported no meaningful changes in several outcomes despite higher NAD-related biomarkers. Examples include skeletal muscle metabolism, insulin sensitivity, and body composition in selected groups.20-22
The evidence therefore supports a narrow claim: some NR and NMN products can raise NAD+ biomarkers. It does not support claims that they reverse aging, create a younger biological age, extend human lifespan, or prevent age-related disease.

NAD+ IV Therapy and Injections: What the Evidence Shows
Injectable or IV NAD+ is not an FDA-approved anti-aging treatment, and current evidence for IV or injected NAD+ as an anti-aging or wellness treatment is limited.23 A 2026 systematic review found no eligible outcomes trials testing intravenous, or IV, NAD+ or intramuscular, or IM, NAD+ for these uses.18
IV therapy delivers a substance through a vein, while an IM injection places it into a muscle. However, that does not necessarily mean the outcomes are better. Despite this, interest in NAD+ IV therapy has grown through wellness-clinic promotion and celebrity coverage.
Claims may include better energy, healthier aging, or improved longevity, but there is little evidence for this. There is also not enough research to show that IV NAD+ is better than an oral supplement.
IV NAD+ may be compounded by traditional pharmacies under FDA’s interim section 503A policy, provided applicable conditions are met.²⁴˒²⁵
Sterile preparation matters because contaminated or poorly prepared injections can cause serious harm. The FDA has reported safety concerns involving compounded injectable NAD+ products made with ingredients unsuitable for sterile drug production.26 Because of this, it’s important to look for PCAB Accreditation through the Accreditation Commission for Health Care (ACHC) or Compounding Pharmacy Accreditation from the National Association of Boards of Pharmacy (NABP).27 These oversight requirements do not make NAD+ an approved anti-aging treatment. A clinic that discloses its pharmacy partner, along with ACHC, PCAB or NABP accreditation, provides an additional quality signal, but accreditation does not prove that the treatment works.

Supporting NAD+ Naturally: Diet, Exercise, and Lifestyle
The body makes NAD+ from dietary precursors, and lifestyle habits are the first-line, lowest-risk way to support NAD+ metabolism. These steps are not proven to raise NAD+ enough to reverse aging. However, they do support broader health outcomes that go far beyond NAD+ alone.
Diet
Vitamin B3 is a group of nutrients that includes niacin and nicotinamide. The body uses these as starting materials for NAD+ production. Tryptophan, an amino acid found in protein-containing foods, is also needed to make NAD+.2,3
Foods that provide vitamin B3 or tryptophan include:
- Fish, poultry, and lean meat
- Peanuts and other legumes
- Whole or fortified grains
- Milk, yogurt, and eggs
- Mushrooms and green vegetables
These foods do not act like a high-dose supplement or infusion. They provide nutrients within a broader eating pattern and help prevent vitamin B3 deficiency.
Fasting and calorie restriction have also been studied in relation to NAD+, sirtuins, and aging biology.28 The evidence is stronger in cells and animals than in humans, and prolonged fasting may be unsafe for some people. Speak to your provider before beginning a fasting or calorie-restrictive diet.
Exercise and sleep
Regular physical activity may also support NAD+ metabolism. Human skeletal muscle research has linked exercise training with activity in the NAD+ salvage pathway, which recycles used NAD+ components so they can be rebuilt.2,29 Beyond NAD+, exercise is one of the best things you can do for your health.
Sleep and the body’s internal clock are connected with metabolism and NAD+-dependent signaling, but there isn’t enough research to connect a sleep schedule with NAD+. However, sleep is another critical part of your overall health.
A cautious lifestyle-first approach includes regular movement, adequate sleep, a balanced diet, avoiding tobacco, and managing blood pressure and other established health risks. Regardless of how they affect NAD+ levels, they are excellent ways to protect your health as you age.
Safety and Who Should Be Cautious
Oral NAD+ precursors appear generally well tolerated in short-term studies, but long-term safety data are limited, and some groups should be cautious. Being tolerated over a few weeks or months is not the same as being proven safe over years of high-dose use.16,17,30
Common reported side effects
Across NAD+ products, reported side effects have included:
- Nausea or stomach discomfort
- Headache or lightheadedness
- Diarrhea
- Muscle cramps
- Skin irritation
The product, dose, route, and other ingredients all affect how someone reacts. IV infusions can add their own issues, including flushing, chest discomfort, abdominal symptoms, vein irritation, or problems related to the sterile procedure.31 Most reported effects have been mild, but if a symptom is severe, persistent, or worrying, stop and check with a clinician rather than pushing through it.
If you have cancer or a history of cancer
Talk with your oncology team before using any NAD+-raising product. The reasoning cuts both ways: NAD+ supports normal DNA repair and cell metabolism, but cancer cells also depend on metabolic pathways, and human evidence has not established whether long-term supplementation is safe across different cancers, treatments, and stages.32-33 This is not a reason to panic if you have taken it, but it is a genuine reason to bring it up with the team that knows your case.
If you have liver disease or are pregnant or breastfeeding
Seek medical advice first. The liver does much of the work of processing nutrients and drugs, so advanced liver disease changes the risk picture. In pregnancy and breastfeeding, the issue is simply missing information: adequate safety studies are lacking, and “not enough evidence” is a reason for caution, not reassurance.34 A clinician can help you weigh it against your specific situation.
If you take medications or have several health conditions
Loop in a clinician or pharmacist before long-term or high-dose use. This matters most when you take prescription medicine or manage more than one condition, because supplement labels may not clearly spell out interactions or your total vitamin B3 exposure across everything you are taking. A quick medication review is low-effort and can catch problems a label will not.

What to Consider
A decision about NAD+ should account for evidence quality, product quality, route, cost, and your own health history. The central question is not just whether a product raises NAD+. It is whether that change produces a benefit that actually matters to you, such as more energy, better health, or a longer independent life, rather than a number on a lab report.
Evidence quality
A biomarker result is an early step, not a final health outcome. Higher blood NAD+ shows that a product affects your metabolism, but it does not show that you will stay independent longer, avoid disease, or live longer.
Evidence gets more useful as it moves from cell and animal studies to randomized human trials and long-term outcomes. Right now, NAD+ research is strongest at the mechanism and biomarker levels, and weakest on the anti-aging outcomes people actually care about. A study should also match the claim: a short trial showing a blood change cannot support a claim about lifespan, and a result in people with a specific metabolic condition may not apply to a healthy adult hoping to age well. When a product’s marketing outruns the strength of its evidence, that gap is the thing to notice.
Supplement quality
Because oral supplements are sold over the counter with no medical gatekeeping, the quality check falls to you. FDA manufacturing rules apply, but quality still varies between products.7,8
A few things make that easier. Look for independent verification from USP or NSF, which checks ingredient identity, strength, and contaminants (though it does not prove the product slows aging). Check that the label states the active ingredient and amount per serving, since proprietary blends hide the actual dose. And treat a hidden manufacturer, disease-treatment claims, or guaranteed anti-aging results as reasons to walk away.
Oral precursor versus IV therapy
Oral NR and NMN have more human biomarker research behind them than IV or injected NAD+ does for aging. That does not make the pills a proven anti-aging treatment, but their effects on NAD-related measurements are at least better documented.
IV or injected NAD+ usually means a wellness-clinic intake and paid sessions, plus the added risks and hassle of sterile preparation and administration, and current evidence does not show that IV delivery produces better anti-aging results than a pill. If a clinic guarantees anti-aging results, promises to reverse aging, or sells drip packages as proven longevity treatment, it is claiming more than the evidence supports. Celebrity use does not change the quality of the data.
Cost and value
Whether NAD+ is worth it depends on what you expect for your money. Paying for a biomarker change is a very different purchase from paying for a treatment proven to improve health or lifespan, and right now only the first one is on offer.
Supplements are an ongoing expense, and supervised infusions can mean repeat clinic charges, with no promised return because the anti-aging benefit is unproven. It is worth weighing that spend against things with a stronger track record: nutritious food, physical activity, medical care, and any treatment you have actually been prescribed.
Lifestyle first
Lifestyle stays the foundation of healthy aging. No supplement replaces exercise, sleep, avoiding tobacco, a sound diet, or managing high blood pressure, diabetes, and other risks. This is not a knock on NAD+ biology; it is just that the stronger, more practical evidence should steer your decisions while the long-term human trials are still incomplete.
When to involve a clinician
Bring in a clinician if you have cancer or a cancer history, advanced liver disease, or are pregnant or breastfeeding, and before any high-dose, long-term, IV, or injected use. A good conversation can cover your medications, what you are hoping to get out of it, the possible downsides, and whether any testing makes sense. One last thing worth knowing: commercial “biological age” tests are not interchangeable, and a change on one of them does not mean your body has actually grown younger.

NAD+ is a legitimate and active area of aging research. Oral precursors such as NR and NMN can raise NAD-related biomarkers, but current human evidence does not prove that they reverse aging, extend lifespan, or prevent age-related disease.
A decision should weigh the quality of the evidence, product testing, cost, route of use, and personal health history. There is no proven anti-aging shortcut. Lifestyle measures remain the lower-risk foundation for healthy aging.
