Transcranial Magnetic Stimulation (TMS) for Obsessive Compulsive Disorder (OCD)
Recruiting
Transcranial magnetic stimulation (TMS) is an FDA-approved, non-invasive form of brain stimulation for OCD. TMS has been shown to be an effective form of treatment in individuals with OCD. By using a form of TMS termed theta-burst stimulation (TBS), we hope that this will result in a more effective treatment by producing faster symptom reduction. We are trialing a novel form of accelerated TMS, where we will deliver ten 10-minute sessions per day, for 5 days. This study will compare the efficacy of TMS for OCD at two distinct brain regions. Participants will be randomized into one of two study groups, and receive stimulation at either the DMPFC or R-OFC. Both regions have been linked to OCD by research. Patients that don't respond to their study region will have the option to receive stimulation at the other region.

Novel Characterization of Sex Specific Biologic Signatures in Valvular Heart Disease
Recruiting
This project aims to validate sex-specific biologic signatures associated with aortic valve disease developed in a large multicenter CMR registry, using unsupervised phenomapping.
We aim to use advanced CMR techniques (MRF, DTI, chemical exchange transfer, and radiomics analysis) to determine advanced CMR predictors of reverse remodeling following aortic valve surgery and develop sex-specific thresholds for risk. Infrastructure developed by this study will enable development of an innovative, s... Read More

AI in Outpatient Practice for Diagnosing Aortic Stenosis and Diastolic Dysfunction
Recruiting
Two recently developed artificial intelligence-enabled electrocardiogram (AI-ECG) models have been developed to detect aortic stenosis (AS) and diastolic dysfunction (DD). AI-ECG for AS has a sensitivity of 78% and specificity of 74%, and AI-ECG for DD has a sensitivity of 83% and specificity of 80%. However, these models have never been prospectively applied to diagnose AS or DD, which may be useful for patients and providers from a diagnostic and prognostic perspective and especially in settin... Long Term Evaluation of Cardiac Arrhythmias After Transcatheter Aortic Valve Implantation -The LOCATE Registry
Recruiting
The purpose of the Long term Evaluation of Cardiac Arrhythmias after Transcatheter Aortic Valve Implantation (LOCATE) Registry is to perform long-term ambulatory monitoring of patients with severe aortic stenosis who undergo trans-catheter aortic valve implantation (TAVI) and develop new onset conduction system abnormalities post-procedure that do not require urgent permanent pacemaker (PPM) implantation. The primary objectives of this study are to assess the incidence of late onset heart block... Read More

Loss of Y Chromosome in Aortic Stenosis
Recruiting
The most common heart valve disease in humans is aortic stenosis which is a critical narrowing of the valve through which the heart has to pump blood to the rest of the body. This condition occurs in 2-3% of adults over 65 years of age and when it progresses to a severe stage leads to heart failure and need for valve replacement procedures (including surgery and catheter-based replacement). Aortic stenosis has a strong male predominance. The purpose of this study is to evaluate whether loss of Y... Read More

Staged Complete Revascularization for Coronary Artery Disease vs Medical Management Alone in Patients With AS Undergoing Transcatheter Aortic Valve Replacement
Recruiting
Patients undergoing transcatheter aortic valve replacement (TAVR) often have concomitant coronary artery disease (CAD) which may adversely affect prognosis. There is uncertainty about the benefits and the optimal timing of revascularization for such patients. There is currently clinical equipoise regarding the management of concomitant CAD in patients undergoing TAVR. Some centers perform routine revascularization with percutaneous coronary intervention (PCI) (either before or after TAVR), while... Read More
