NAD+ Benefits: What Does the Science Say?

If you have searched for NAD+ benefits, you have probably found bold promises next to careful science, and no clear way to tell them apart. NAD+, short for nicotinamide adenine dinucleotide,1 is a coenzyme found in every cell in your body. A coenzyme is a helper molecule that lets your cells run essential chemical reactions.2 NAD+ helps convert the food you eat into usable energy, supports the repair of damaged DNA, and helps your cells respond to stress. These roles are well established in basic biology.

Here is the catch. The fact that NAD+ is essential inside cells does not prove that taking a NAD+ supplement will make you feel more energetic, lose weight, think more clearly, or age more slowly. Those are separate questions, and they need separate evidence.

Regulatory Status

Oral NAD+ precursor products such as NMN, NR, and NAM are sold as dietary supplements under the Dietary Supplement Health and Education Act of 1994 (DSHEA).3 This means they do not receive FDA premarket approval the way drugs do. The manufacturer, not the FDA, is responsible for confirming that a supplement is safe and labeled accurately before it reaches the market. Supplement makers must still follow federal manufacturing rules under 21 CFR Part 111,4 so not FDA-approved does not mean wholly unregulated. Independent testing programs such as USP or NSF can add a layer of quality verification, though they do not test whether a product works.5

What NAD+ Actually Does in the Body

NAD+ is a coenzyme your cells use to turn food into energy, repair damaged DNA, and respond to stress.1,2,6,7 It works as a shuttle for tiny charged particles called electrons, and it acts as fuel for a set of repair and signaling enzymes. These functions are essential and well documented, but they describe how cells work, not what a supplement will do for you.

NAD+ is often marketed like it’s the ingredient itself, when really it’s the helper. Our complete guide to NAD+ explains the difference, and why that distinction matters for every claim below. 

The most basic job of NAD+ is energy production. Your cells make a molecule called ATP, short for adenosine triphosphate, which is the main energy currency they spend to do work. To make ATP, cells pass electrons through a series of reactions, and NAD+ carries those electrons.1 When NAD+ picks up an electron, it becomes NADH. The pair, NAD+ and NADH, cycles back and forth constantly, delivering electrons to the mitochondria, the structures inside cells where most ATP is made.7 Without this cycle, energy production stalls.

NAD+ also acts as raw material for two important groups of enzymes. The first are sirtuins, enzymes that help regulate how cells respond to stress and manage their internal maintenance.8,9 The second are PARP enzymes, which stands for poly(ADP-ribose) polymerases, and these help detect and repair breaks in DNA.10 Both sirtuins and PARP consume NAD+ as they work, which links your cell’s NAD+ supply to its ability to repair itself and handle stress.8-10

Research shows that NAD+ levels tend to decline with age in many tissues.11 This decline is one reason NAD+ has become a focus of aging research. If cells have less NAD+ over time, the reasoning goes, then their energy production and repair systems may work less efficiently.8,11 That is a plausible hypothesis, and it explains why scientists are interested.

But interest is not proof. A measurable decline in NAD+ with age does not automatically mean that raising NAD+ with a supplement will slow aging, restore energy, or improve health. The gap between an established biological role and a proven human benefit is exactly where most NAD+ marketing overreaches. Everything that follows in this article is built to help you tell those two things apart, so you can judge each claim on its own evidence rather than on biology alone.

How to Read NAD+ Benefit Claims

NAD+ benefit claims fall into four tiers, and knowing which tier a claim sits in tells you how much to trust it. The tiers are: an established biological role, biomarker evidence, possible clinical benefit, and unproven claim. Most advertising quietly slides from the first tier to promises that belong in the third or fourth.

The single most important idea is this. Having an elevated level of a NAD-related metabolite does not equate to improving a health outcome you can feel or measure clinically. A supplement can reliably raise blood NAD levels and still fail to change your energy, weight, blood sugar, or lifespan.12-14 Biomarkers are easy to move. Outcomes are hard to prove.

Energy and Fatigue

NAD+ supports the reactions that produce cellular energy, but that does not prove supplements fix everyday tiredness, and the evidence for general use is weak. NAD+ being part of energy production is a biological fact, not a demonstrated effect of taking a pill. Feeling tired usually has many causes, including sleep, stress, nutrition, and underlying medical conditions, and low cellular NAD+ has not been shown to be the reason healthy adults feel fatigued.

Healthy Aging and Longevity

NAD+ is central to the biology of aging, and precursors can raise NAD-related biomarkers, but no human evidence proves that supplements reverse aging or extend lifespan. The aging connection is real at the level of cell biology, where NAD+ declines with age and supports repair systems.6-8 That is why researchers study it. It is not evidence that any product will help you live longer or look younger.

Animal studies have shown some effects on markers of aging, but results in mice do not reliably predict results in people.17-20 Human trials so far have measured biomarkers, not lifespan or age reversal.11,20 For the full picture, see “NAD+ for Anti-Aging: What the Research Says.”

Metabolic Health, Blood Sugar, and Weight

NAD+ is involved in how your body processes energy and nutrients, but human supplement trials have not consistently improved blood sugar control, insulin markers, or blood fats. In plain terms, NMN can raise your blood NAD levels while leaving the numbers that actually matter for metabolic health largely unchanged.13,21-23 This is one of the clearest examples in the whole field of a biomarker moving without an outcome following.

Meta-analyses, which pool results from multiple randomized controlled trials, have looked at NMN supplementation in adults.22-24 These reviews found that while NMN reliably increased blood NAD levels, it did not produce significant improvements in fasting glucose, fasting insulin, HbA1c (a measure of average blood sugar over about three months), HOMA-IR (an estimate of insulin resistance), or lipid profiles compared with placebo.22-24 In other words, the biomarker rose, and the clinically meaningful outcomes did not.

One review went further and concluded that the metabolic benefits of NMN may be exaggerated relative to what the human trial data actually support.24 That is a notable statement from within the research literature itself, not from critics outside it.

This matters directly for the most common reason people try NAD+ for metabolism, which is weight. There is a reasonable biological story here; NAD+ participates in the pathways that handle fuel, so studying it in metabolism makes scientific sense.7 However, current evidence does not support NAD+ or NMN for weight loss.14,22-24 

The honest takeaway is measured. If you have diabetes, prediabetes, high cholesterol, or a weight goal, NAD+ supplements are not a proven tool, and they should not replace treatments and lifestyle changes with real evidence behind them.

Brain, Cognition, and Neurologic Research

NAD+ is relevant to how brain cells produce energy and is being studied in neurologic conditions, but it is not proven to improve general thinking or to treat neurologic disease. Brain cells are energy-hungry, so NAD+ biology is biologically relevant to the brain.7 However, there is no reliable human evidence that NAD+ supplements sharpen memory, clear brain fog, or boost cognition in healthy people.

The clearest example of this early research is the NADPARK trial, a phase I study in people with Parkinson’s disease.25 Phase I means the earliest stage of human testing, focused mainly on safety and on whether a treatment does what it is supposed to biologically. In NADPARK, the precursor nicotinamide riboside was well tolerated and raised NAD levels measured in the brain.25 Mild clinical improvement was seen only in a subgroup of participants described as responders, not across the whole group.25

NAD+ is a legitimate area of neurologic research, and early trials are ongoing. If you are considering NAD+ for a neurologic condition, speak with your provider. They can help decide if it is safe for you to take.

Heart and Vascular Health

Heart and blood vessel cells depend heavily on steady energy production, which makes NAD+ biologically relevant to cardiovascular health. The heart is one of the most energy-demanding organs, and NAD+ helps power the reactions that keep it working.7

Some early human research has looked at whether NAD+ precursors affect measures such as blood pressure or arterial stiffness, meaning how flexible your arteries are.11,15,26 These studies are small and preliminary, and they have not established that supplements meaningfully improve vascular function or heart health in a way that changes clinical outcomes.11,26

This is a plausible area of research, but at this time NAD+ has not been shown to prevent or treat cardiovascular disease.14 

Skin, Hair, and Beauty

NAD+ biology is relevant to cellular repair and stress responses, but that does not prove oral NAD+ supplements smooth wrinkles, boost collagen, slow skin aging, or grow hair. The connection between NAD+ and cellular maintenance is real at the biology level.27,28 It is not evidence that swallowing a NAD+ precursor changes how your skin or hair looks.

There is an important source of confusion here. Niacinamide, a form of vitamin B3, is studied and used in topical skincare, meaning products applied directly to the skin. That research is about a topical ingredient with its own uses and its own evidence base.29 It is not the same as evidence for oral NAD+ boosters, which are different products taken a different way for different reasons. Marketing sometimes blurs the two, but they are different supplements.

For oral NAD+ supplements and beauty outcomes, there is very little human evidence.14

Exercise and Physical Performance

NAD+ may influence mitochondrial function and how muscles use energy, but human performance findings are mixed and too weak to support broad claims. The biological link between NAD+ and muscle energy is real, which is why researchers have tested precursors in exercise studies.6,7 The results have not shown a consistent, meaningful benefit for endurance or performance in general.

The strongest specific signal worth mentioning honestly comes from a trial in older adults, where NMN taken in the afternoon was associated with improved lower-limb function.30 That is a narrow finding in a specific group at a specific dosing time. It does not establish that NAD+ precursors improve endurance, strength, or athletic performance for the general population.

Exercise remains the best way to improve physical fitness.

NAD+ IV Therapy and Injections

Intravenous or injected NAD+ is popular in wellness settings, but strong human outcomes evidence is limited, and a 2026 systematic review found no eligible outcomes trials of IV or intramuscular NAD+ itself for wellness or anti-aging.14 In other words, the most heavily marketed and expensive form of NAD+ has the least completed outcomes evidence behind it.

IV or injected NAD+ is usually a compounded sterile drug preparation, meaning it is mixed by a pharmacy for a specific use rather than being an FDA-approved medication.31-34 If you are going to get an infusion, how it was prepared matters a great deal, because a contaminated or poorly made sterile product can cause serious harm. 

This is where accreditation earns its place. Verifiable state licensure, a clearly disclosed pharmacy partner, and voluntary accreditation such as PCAB (the Pharmacy Compounding Accreditation Board, administered by ACHC) or NABP signal that a pharmacy has met safety and quality standards beyond the legal minimum.35 Those are real signals worth looking for, and their absence is a reason to be cautious. What they do not tell you is whether the treatment works: an impeccably compounded NAD+ infusion is still an infusion with no completed wellness or anti-aging outcomes trials behind it.

There are also procedure-related risks to weigh. Any IV or injection carries risks such as infection, vein irritation, and reactions at the injection site, and infusion reactions have been reported with NAD. Given the lack of completed wellness outcomes trials, these risks and costs deserve careful thought.

Safety and Who Should Be Cautious

Oral NAD+ precursors appear generally well tolerated in short-term research, but long-term safety data are limited, and some groups should be cautious. Reported side effects in trials have been generally mild and have included nausea, stomach discomfort, and headache, along with lightheadedness, diarrhea, muscle cramps, and skin irritation.11,14,15,36,37 Most studies have run for weeks to a few months, so effects over years are not well characterized.

Also, at very high doses niacin can cause liver problems and other side effects.38

Talk to your provider first if you:

  • Are pregnant or breastfeeding
  • Have cancer or a history of cancer
  • Have advanced liver disease

What to Consider

Which Benefit Do You Actually Want?

Start by naming the specific outcome you are after, then check it against the evidence grid above. Ask if what you want is more energy, better metabolism, sharper thinking, or slower aging, or if your interest is simply understanding what NAD+ does inside cells. In many cases, you may be able to accomplish some of these goals with lifestyle changes, such as sleep, diet, and exercise.

Set Realistic Expectations

Even if a supplement raises your NAD-related biomarkers, that change does not guarantee more energy, weight loss, better cognition, slower aging, disease prevention, or a longer life. The throughline of the entire evidence base is that biomarkers moved while outcomes usually did not.12-15,25 Expect, at most, a measurable blood change with uncertain personal meaning. If a product promises a felt or life-changing result, the promise is ahead of the science.

Check Supplement Quality

Oral NAD+, NMN, NR, and NAM products are dietary supplements, not FDA-approved drugs, so quality can vary between brands and even between batches. Look for meaningful independent verification such as USP or NSF, which check identity and purity, though not effectiveness.5 If you have a health condition or take medications, discuss any product with your clinician or pharmacist before starting.

Compare Oral Supplements With IV or Injected NAD+

Oral products are available over the counter. IV and injected NAD+ typically involve a wellness-clinic intake and a paid session and carry procedure-related risks.14 At the time of writing there is not enough research yet to know if either one has a benefit. Weigh Cost Against Unproven Benefit

Both supplements and clinic sessions tend to be recurring costs, and it is fair to ask what you are buying. When the clinical benefit is uncertain and the marketed benefit is unproven, the money is being spent on a possibility, not a result. Treat broad promises about reversing aging, curing fatigue, or fixing metabolism as warning signs that claims are running ahead of evidence.

Start With Lifestyle

Food, sleep, regular exercise, and limiting alcohol are great, lower-risk ways to support your body’s normal NAD+ metabolism, and they are well established for long-term health overall. These cost little and carry benefits that supplements have not matched in trials.

Conclusion

NAD+ is essential for cellular energy, metabolism, DNA repair, stress response, and normal cell function, and oral precursors can raise NAD-related biomarkers in people.6,7 That much is well supported. What is not supported is the leap from those facts to promises about reversing aging, curing fatigue, driving weight loss, sharpening cognition, or extending life. Across human trials, biomarkers often rose while meaningful outcomes stayed mixed or null.14

The evidence is strongest for biological function and weakest for felt or clinical benefits. So put lifestyle and a clinician conversation before any supplement, and judge each claim by the tier of evidence behind it. When a product makes a broad benefit promise, treat that as a reason to slow down and ask for proof, not a reason to buy.

Frequently Asked Questions

NAD+’s established benefits are biological. It depends on what you mean by work. Yes, oral precursors such as NMN and NR can raise NAD-related biomarkers in humans.25 This is one of the better-documented effects in the field. But raising a biomarker is not proof of a clinical benefit, and in metabolic trials the NAD level rose while outcomes did not.13,21-25
